BACKGROUND
In 2011, pediatric hematopoietic stem cell transplant (HSCT) patients were moved from an older hospital to a new children’s hospital. To minimize bacterial growth in the new hospital’s water during construction, the plumbing system was flushed and disinfected before occupancy. However, 6 months after occupancy, an increased incidence of rapidly growing mycobacteria (RGM) was detected in clinical cultures. Over 10 months, 15 pediatric HSCT patients were infected, while no pediatric HSCT patients had been infected in the preceding 12 months.
OBJECTIVETo determine the cause of the outbreak and to interrupt patient acquisition of RGM.
METHODSWater samples were collected from water entering the hospital and from drinking water and ice machines (DWIMs) from the old and new hospitals. Total heterotrophic plate counts (HPCs, CFU/mL) of water were undertaken, and select isolates were identified as RGM.
RESULTSThe cause of the outbreak was increased bacterial levels in the water (including RGM) in the DWIMs in the new (2011) hospital. Tests revealed higher HPCs in drinking water and ice from the DWIMs in the new hospital than in the DWIMs in the old hospital. Ultimately, HPCs were reduced by several different interventions.
CONCLUSIONIn response to an RGM outbreak, HSCT patients were banned from ingesting DWIM ice and water and bottled water was provided. Since this interverntion 4 years ago, no additional RGM isolates have been identified in HSCT patient cultures. Our measures to reduce HPCs to goal levels in drinking water from DWIMs were successful, but the HPCs for ice have not consistently reached the goal of <500 CFU/mL.

Summary
Introduction
The stakeholders involved in management of land and carbon (C) are diverse. Farmers and foresters are concerned with plants and management practices that are most likely to sustain profits. The opportunity to sell C sequestration credits adds a new dimension to production strategies. Land managers may be asking questions, such as how tillage and fertilizer practices in a specific location affect C storage and crop yields. Regional planners and governing bodies may have the opportunity to influence where and how cultivation occurs and interacts with other land uses and industries. They may ask questions related to how crops can be distributed across a landscape to achieve multiple goals that reflect local priorities (water quality, scenic views, traditional lifestyles, tax revenues, etc.). At state and national levels, there are requirements to manage human activities to comply with land, water, and air-emission regulations as well as policy objectives such as job creation and energy security. Decision makers at these levels may desire guidance on how the interactions of policy options provide incentives or disincentives for certain land-use practices and resulting environmental and socioeconomic impacts. Many decision makers are most interested in how scientific information can be used to guide land-use practices in the near term, typically one to five years. However, the scientific information may derive from data measured at entirely different scales or locations and in time spans that range from decades to centuries. With rising attention to global markets and climate change, managers are concerned about how changes in their region are affected by global processes. National and regional decision makers want to know how their choices affect productivity, incomes, C and nutrient cycles, and other development goals. There needs to be a better match between the diverse needs of managers and the information provided by scientific analysis and models.
An Outbreak of Bacteremias Associated With Mycobacterium mucogenicum in a Hospital Water Supply

Objective:
To investigate and determine the cause of an outbreak of Mycobacterium mucogenicum bacteremias in bone marrow transplant (BMT) and oncology patients.
Design:Case–control study and culturing of hospital water sources. Isolates were typed using molecular methods.
Setting:University-affiliated, tertiary-care medical center.
Patients:Case-patients were adult and pediatric BMT patients or hematopoietic stem cell transplant (BMT) (n = 5) and oncology (n = 1) patients who were diagnosed as having M. mucogenicum bacteremia during the study period of August through November 1998. Two control-patients were selected for each case-patient matched by age, time of hospitalization, inpatient unit, and type of patient (BMT or oncology).
Results:There were no significant differences between case-patients and control-patients regarding intravenous products received or procedures performed, frequency of bathing, neutropenia, or steroid use. Nontuberculous mycobacteria were isolated from several water sources at the medical center including tap water from sinks and showerheads, the hospital hot water source, and the city water supply to the hospital. Analysis by multilocus enzyme electrophoresis and randomly amplified polymorphic DNA showed a match between one patient's blood isolate and an isolate from shower water from that patient's prior hospital room.
Conclusions:The cause of the outbreak seemed to be water contamination of central venous catheters (CVCs) during bathing. A recommendation in early 2001 that CVCs be protected from water during bathing was followed by no M. mucogenicum bacteremias during the second half of 2001, only one in 2002, and none at all during 2003.

We examined whether responder type groups reflecting patterns of hemodynamic reactivity might also differ in recovery responses. Cardiac output (CO), total peripheral resistance (TPR), systolic and diastolic blood pressure, heart rate, and Heather index were assessed at rest and during speech and cold pressor tasks in young adults. Participants (n = 152) were classified as myocardial, vascular, or mixed-mild responders based on CO and TPR responses to speech presentation. Vascular responders exhibited slower CO and TPR speech recovery than the myocardial and/or mixed-mild groups. Responder type differences in reactivity showed limited task-generalizability. The sustained vascular response pattern of the vascular group is consistent with that seen in hypertension. In light of associations of heightened TPR with markers of disease risk, this suggests potentially negative health implications for vascular responders.
